The case inquiry form collects the clinic or practice name, contact person, phone number, email, inquiry region, services of interest, preferred timeline, and inquiry details.

Do not enter or attach patient names, dates of birth, chart numbers, photographs, STL files, or any other identifiable health information in the initial inquiry.

2. How we use information

We use the information only to identify the inquirer, review the request, provide service and scheduling information, and communicate about the inquiry.

3. Retention

Inquiry records are retained for the period reasonably needed to complete follow-up, comply with applicable law, and address disputes. Records are securely deleted when no longer required.

4. Service providers and international processing

We may use Google Sheets and email delivery services to receive inquiries and send notifications. Information may be processed on servers outside your country, subject to each provider’s security and privacy practices.

5. Your choices and rights
